Wellness Visit vs. Physical

Wellness Visit vs. Physical: Decoding Your Medicare Doctor's Appointments

Confused about your Medicare appointments? You're not alone! Knowing the difference between an Annual Wellness Visit, an Annual Physical, and a standard Office Visit is the key to managing your health and avoiding surprise bills.

Let's break down what each visit is for and what it costs.

The Medicare Annual Wellness Visit (AWV)

This is a yearly appointment to create a personalized plan to keep you healthy. Think of it as a strategy session for your health. It is NOT a head-to-toe physical exam.

🎯 The Goal: To focus on preventive care and create a health plan for the year ahead.

✅ What Happens During the Visit?

  • You'll fill out a "Health Risk Assessment" questionnaire.

  • Your doctor will review your medical history and medications.

  • You'll get a screening for cognitive health (like memory).

  • Your height, weight, and blood pressure are checked.

  • Your doctor creates a personalized prevention schedule for you (e.g., recommends future cancer screenings or vaccinations).

💰 Cost with Original Medicare:

$0! This yearly visit is covered 100% by Medicare Part B. There is no copay or deductible.

(Note: If your doctor performs additional tests or addresses a new health problem during this visit, you may have a copay/coinsurance for those extra services.)

The Annual Physical Exam

This is the traditional, hands-on check-up that most people think of when they hear the word "physical."

🎯 The Goal: A comprehensive, head-to-toe examination of your current health status.

✅ What Happens During the Visit?

  • Your doctor will perform a full physical examination (listening to your heart and lungs, checking your abdomen, etc.).

  • Vital signs are checked thoroughly.

  • Your doctor may order screening tests like blood work (cholesterol, blood sugar).

  • You can discuss any and all health concerns.

💰 Cost with Original Medicare:

NOT COVERED. Original Medicare does not cover routine physical exams. If you ask for a "physical," you will likely have to pay 100% of the cost out-of-pocket.

(Note: Some Medicare Advantage (Part C) plans may offer coverage for annual physicals. Check with your plan provider directly.)

The Office Visit (or "Sick Visit")

This is an appointment you schedule to address a specific health problem.

🎯 The Goal: To diagnose and treat a new or ongoing health issue, symptom, or injury.

✅ What Happens During the Visit?

  • You'll discuss your specific symptoms with your doctor.

  • The exam will focus on the problem area (e.g., looking at a rash, listening to a cough).

  • Your doctor will provide a diagnosis and a treatment plan.

  • Prescriptions may be ordered or adjusted.

💰 Cost with Original Medicare:

You will pay your standard cost-sharing. This includes your Part B deductible and then a 20% coinsurance for the visit. Copayments may also apply.

The Bottom Line: Be Clear When You Book!

To avoid unexpected bills, be very specific when you schedule your appointment.

  • If you want the free preventive visit, say: "I'd like to schedule my annual Medicare Wellness Visit."

  • If you have a health problem, say: "I need to see the doctor for [your specific issue]."

Confirming the type of visit upfront ensures you get the care you need without a surprise on your bill.
